How to use the Questionnaire Builder agent skill
Updated · written with AI from the project's own documentation and checked against our catalogue data
The to-questionnaire skill is an agent skill for Claude that turns a decision you can't settle alone into a structured Markdown questionnaire for someone else to fill in. It is aimed at managers, leads and engineers who need facts or opinions from a colleague or client before deciding. After this guide you will know what the skill does, how to install it, how to trigger it with a natural request, and what to check before sending the output.
Quick start
- 1Download and unzip. Download the ZIP from this skill page and unzip it into ~/.claude/skills/ or a project's .claude/skills/ folder.
- 2Or upload in the Claude apps. If you use the Claude apps, go to Settings → Capabilities → Skills and upload the skill.
- 3Ask for the skill by name. Describe a decision you can't make alone and ask for a questionnaire, naming the skill if you like.
- 4Answer the two interview questions. Tell the agent who the recipient is and what specific decisions or facts you need back.
- 5Find the saved file. Look in your current directory for to-questionnaire-<slug>.md; the agent reports the path.
- 6Check coverage. Confirm every item you named is covered by a question and the template sections are present.
- 7Send it. Share the file by email, a shared document or a meeting, and ask the recipient to flag anything unsure.
What the skill makes the agent do
The skill is built around one idea: the agent grills the send, not the subject. It asks you briefly about who the questionnaire goes to and what you need back, because you can answer those questions. It does not ask you to explain the topic itself, since the recipient holds the knowledge you lack.
The agent then drafts questions aimed at the gap between what the recipient knows and what you need. The document is a discovery questionnaire, with questions ordered most-important-first because you may only get one pass at the answers.
- Asks who the recipient is, their role, expertise and relationship to you, in one exchange.
- Asks which specific decisions or facts you need back, in one exchange.
- Writes the questionnaire to a file named to-questionnaire-<slug>.md in your current directory and reports the path.
Install the skill from the ZIP
Download the ZIP from the skill's page on this directory and unzip it. The skill folder contains agents/openai.yaml and SKILL.md. To make it available across your projects, place the unzipped skill in ~/.claude/skills/. To limit it to one project, place it in that project's .claude/skills/ folder.
If you use the Claude apps instead of the file system, you can upload the skill under Settings → Capabilities → Skills.
- Personal install: ~/.claude/skills/
- Project install: .claude/skills/ inside the project folder
- Claude apps: Settings → Capabilities → Skills, then upload
Trigger the skill with a natural request
The skill is set to disable automatic model invocation, so it does not start on its own when a topic comes up. You need to ask for it directly, in plain words that describe a decision you can't make alone and a person you need input from.
Name the skill or describe the situation clearly. Once triggered, the agent will start with its two short interview questions before drafting anything.
- Use the questionnaire skill to get requirements from our database engineer before we pick a vendor.
- I can't decide on the refund policy alone. Make a questionnaire for the finance lead.
- Turn this open question about client data retention into a questionnaire I can send.
Review and send the questionnaire
Open the saved file, check that each question you named in the interview is covered, and send it by email, a shared document or a meeting agenda. The file follows a fixed template: a title, a Purpose line, a From and To line, a Context section, a How to answer section, themed question sections, and an Anything else closing section.
Each question is a single idea with an empty answer stub beneath it. Some questions include a short why-this-matters line where the question could be misread.
- Confirm the Purpose line states the decision the answers will feed.
- Confirm the How to answer section gives a deadline and rough effort.
- Confirm the recipient is told that partial answers and I don't know are useful.
Safety and what the skill does not do
The skill does not run scripts and does not use the network. It creates a plain Markdown file in your current directory. That file has no access controls, so anyone who receives or finds it can read it, and it can be forwarded or stored without your knowledge.
Do not use the skill for topics where a written document could leak sensitive or confidential information. Also do not use it for simple questions you can answer yourself or look up.
- No scripts are run and no network use is declared.
- The output is plain Markdown with no access controls.
- Review the file before sharing it, especially if it names people or contains sensitive context.
What people use it for
- A manager collecting technical constraints from a senior engineer before approving a project.
- An engineer gathering requirements from a client contact through a written questionnaire.
- A lead turning a long, tangled email thread into one reusable question document for a subject-matter expert.
- Preparing an agenda of questions to work through together in a planning meeting.
Frequently asked questions
How do I install the to-questionnaire skill?
Download the ZIP from the skill page and unzip it into ~/.claude/skills/ for personal use, or into a project's .claude/skills/ folder for one project. In the Claude apps, you can instead upload it under Settings → Capabilities → Skills.
Does the to-questionnaire skill cost money or what is its licence?
The skill is listed under the MIT licence. The source does not state any price, so check the directory page for any pricing details it provides.
What does the skill require to run?
The listing shows no specific requirements, runs no scripts and uses no network. It works by creating a Markdown file in your current directory.
Where is the questionnaire saved?
It is saved as to-questionnaire-<slug>.md in your current directory, with the slug taken from the topic. The agent reports the full path when it finishes.
Why won't the skill start on its own?
The skill has automatic model invocation disabled, so you need to ask for it directly with a clear request about a decision and a recipient.
Is it safe for confidential topics?
Be cautious. The file is plain Markdown with no access controls, so it can be forwarded or stored unexpectedly. Avoid the skill for sensitive or confidential matters.

Our verdict
Use this skill if you need structured input from a specific expert and want a clear, reusable question document instead of a long email thread. Skip it for questions you can answer or look up yourself, and for sensitive topics where a plain local file could be shared or stored without your control.
